Registration

The registration information on this page is applicable to any persons residing in Arizona who are over the age of eighteen and interested in officiating lacrosse games. Follow the instructions below to register.

Official's Costs

ALOA Membership (Annual):	$50
US Lacrosse Membership (Annual):	$50
Uniform Cost (One Time):	$120

1. ALOA:  Membership Registration

A.  Open the ALOA Membership Registration form, fill it out, print it and sign it.

B.  Scan the signed form and email it to mwmcelfre@cox.net or mail it to the address provided on the form or hand it in at a classroom training clinic.

C.  Pay your $50 dues using paypal

2. US Lacrosse: Membership Registration

Youth Officials: US Lacrosse membership as a lacrosse official is required for youth lacrosse officials.  ALOA officials are required to register as officials with US Lacrosse as they receive additional insurance benefits.

You can obtain a US Lacrosse membership online using the link below. Visit the 'Membership Information' section and choose to 'Apply or Renew Online':

Be sure to select 'Boy's Lacrosse Official' as a membership type in addition to any other applicable types. Selecting 'Boy's Lacrosse Official' provides you insurance as an official of youth lacrosse games.
